While most hospitals have made patient safety a priority – there are instances when hospital errors occur resulting in infections, mistakes, injury and even death. A National Post inquiry on January 16, 2015 revealed that 23,000 patients die because of preventable adverse events in acute care hospitals alone. This inquiry also stated that 70,000 serious preventable injuries occur as a result of erroneous treatments every year in Canada and many errors go unreported. Some studies estimate that hospital errors are the third leading cause of death in North America accentuating the need for patients to protect themselves and their families.

Human error is rarely intentional in the care of a patient. But the refusal to make things right, and to own up to the error and any resulting physical and emotional injury, is an intentional act.
